Please add the IAX and IAX2 VoIP protocol for the Asterisk PBX to the QOS protocol selector pull-down. 

IAX uses a single UDP port for Source / Destination: 
IAX uses UDP/5036 
IAX2 uses UDP/4569

You will be able to add this yourself in Alchemy-pre5.2.
