1.  Create a new empty win32 exe project. 
2.  In the directory created for the project by msvc++ create two
    subfolders. Name one 'Source' and the other 'Resources'.
3.  Put the *.cpp and *.h (not RESOURCE.h) into the 'Source' folder.
4.  Put the ICON1.ICO file into the 'Resources' folder.
5.  Add the source files to the project by selecting 'Project' from
    the main menu and then 'Add to Project' and 'Files'. Locate and 
    load *.cpp,*.h files and ICON1.ICO.
6.  Select 'Insert' from the main menu and select 'Resource' from
    the options. Choose 'Dialog' from the list of options shown.
7.  Save the generated resource file (Script.rc) to the 'Resources'
    folder you created earlier.
8.  Use the visual resource interface to design the dialog and drop
    controls and position them where you want. Make sure the dialog
    box id is IDD_DIALOG1.
9.  'Insert' an accelerator table resource and call it 
    'IDR_ACCELERATOR1'.
10  'Import' the ICON1.ICO file from the resources folder. This is 
    done by 'Insert' and then selecting 'Resource'. In the list that
    appears, select icon and then select 'import'. Make sure its id 
    is IDI_ICON1.
11. Build the application.
